PurposeThe Director of Referral Coordination and Navigation, Nursing leads the strategy and daily operations behind seamless patient transitions, efficient referrals, and integrated navigation services across a complex health system. Partnering with leaders in Patient Access, Health Information Management, and Social Work, this role ensures patients move smoothly through every step of their care journey.
