And we do it all with heart, each and every day.Do you have a passion for engaging with people and helping them on their journey to betterhealth Discover a meaningful role with the opportunity to connect, educate, and close gaps in care, emphasizing keeping people safe and well at home.Signify Health, part of CVS Health®, is hiring full-time cliniciansto conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial health plan members.In this role, you will:Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area (majority of visits scheduled weekdays between 8 a.m. and 7 p.m.)Provide 40 hours of availability weekly, consistently providing 160+ capacity units per monthExecute a strong visit completion rate, consistently pacing 110+ completed visits per monthOther role expectations and requirements:Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)Consistently engage with regular manager, team and clinical meetingsBe accountable to Signify Health operational, clinical, and member experience key performance indicators and metricsConduct Video Visits as needed (maintain training, affiliation, and compliance with Video Visit requirements, regular training required)Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)Maintain all required supplies and associated diagnostic equipment related to DPS offeringsMaintain active state licenses in coverage areasMaintain board certification (as applicable)Maintain multi-state licenses (preferred)Experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)Be flexible to travel locally and within licensed states. May require hotel stay, car rental, and/or flight up to 20% annuallyAbility to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies (including, but not limited to, iPad, medical supplies and scale), requiredAccommodate field observations and/or “ridealong” experiences, as requestedDuring our In-Home Health Evaluations (and other associated follow-up visits), you’ll meet people where they are and re-engage them with their health care during an approximately 45-minute pre-scheduled visit.The visit includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.). You will be provided a pre-populated, patient-specific form for each evaluation and are not required to prescribe medicine, order lab tests, or alter people’s current treatment regimen.Discover the stability of becoming a full-time Signifier.
