Essential duties and responsibilities of the Memory Support Program Manager include, but are not limited to: Keeping the HumanGood Memory Support Program Philosophy at the core of all planning and implementation of activities and care to meet the specific needs of residentsCollaborating with Clinical partners to coordinate resident moves and transfers within the Memory Support departmentAssure that resident profile is completed (prior to move-in preferable; no later than 3 business days after move-in) and shared with team members;Conducting orientation and promoting support and/or educational groups for residents and family membersCoordinates with Director of Wellness and Assisted Living when significant changes of residents occur which may lead to level of care change or adjustment;Preparing and updating Level of Care Transfer Notices and Service Plans for Memory Support residents in collaboration with the Director of Wellness and Assisted LivingMaintaining readiness for licensing and accreditation surveysCompleting required state training programs for Memory SupportPerforming other critical duties and responsibilities as required Annual Salary: $70,000 - $80,000 (DOE) To be successful in the role, you would have 1 - 2 years related experienced related to dementia care'Current LPN license in the state to which you are applyingCurrent CPR LicenseMinimum 1 year experience supervising team members for success, direction, and meeting resident needsCurrent or prior related experience or any equivalent combination of training and experience which provides the required skills, knowledge and abilities.
