Maintain current knowledge on side effects and symptoms related to vascular and cardiac therapeutic regimens. Maintain current knowledge base on complications associated with surgery for vascular patients. Assist physicians in the health maintenance of patients in a complex disease state. Patient navigation and care coordination for vascular clinic patients. Assist doctors in performing physical tests and prescribed tests. Help to conduct and interpret laboratory test results and help to interpret results. Call patients with abnormal results and treatment plans in a timely fashion. Educate and inform patients of the best disease prevention and control methods. Work with doctors and other departments whenever a patient is undergoing surgery. Monitor/manage, screen telephone requests for assistance by urgency of the existing patient base. Assist providers in refilling chronic medications per protocols. Interview and assess patients for medical interventions in the office and on the telephone. Initiate medical interventions based on a symptom-focused protocol or guideline.
