We are a comprehensive team comprised of RNs, Physical Therapists, LPNs, LNAs, SLPs, Social Workers and administrative support, all collaborating together to support patients to remain in living in the setting of their choice, their home.The Community Health Nurse, LPN will be responsible for the delivery of basic nursing care to patients in a home care settings, under minimal clinical supervision, as directed by a Registered Nurse, and in accordance with an established nursing plan of care.The general territory for this position is Burlington, Winooski and South Burlington. At times travel may be required outside of assigned territory but a general service area is assigned and part of typical travel. All mileage is reimbursed at the federal gas rate (.65c/mi)!Qualifications:Accredited School of Nursing LPN ProgramTwo or more years of directly related home/public health, and/or hospice experienceCurrent LPN licensure or compact licensure recognized by the State of Vermont requiredPrevious home care desired.Essential Physical Requirements:1. Must walk, sit, stand, bend, lift and move intermittently during working hours2. Must have the ability to push, pull and lift from the floor, up to a total of 50 pounds and be able to move with the weight, as to organization’s standards3. Climbing-be able to navigate stairs of varying heights and degrees4. Balancing-be able to maintain balance5. Kneeling, crouching, stooping, twisting, reaching below waist level, reaching overhead and reaching torso level6. Manual finger dexterity-will need dexterity to perform duties and use equipment7. Department Description: High Technology nursing provides in-home nursing care to adults and children with medically complex conditions that require 24 hour care, specialized skills, or high technology medical equipment.Skills could include ventilator monitoring and care, tracheostomy care, use and care of feeding tubes, and other technical procedures; administration of medications, assisting with general exercise and specific physical therapy regimens; family teaching and support, exercise and physical therapy needs, medical procedures they can do at home, and methods of carrying out daily living activities. Experience with these skills is preferred, but not required.There is often a need to work evenings, weekends, and holidays. We have both a full time day and full time night position open!Although you are in the field 1x1 with your patient your team and manager are a phone call away, you are an integral part of an autonomous, high functioning, collaborative team where our nurses thrive.
