ESSENTIAL DUTIES AND RESPONSIBILITIESDevelop and maintain existing accounts through personal visits and follow-up on a systematic basis. Collaboration with interdisciplinary teams ensures coordinated care, including transfers, discharges, and addressing psychosocial and spiritual needs.
