How youll contribute An RN Case Manager who excels in this role: Coordinates patient care to ensure continuity, optimal outcomes, satisfaction, and cost-effective treatment.Collaborates with patients, families, and the healthcare team to support effective discharge planning and resource referrals.Addresses concerns from patients, families, and payors while monitoring satisfaction and care quality.
