Job Description:The Social Work Care Manager I, utilizes clinical expertise to perform psychosocial assessments, develop and implement care plans in collaboration with the appropriate care team, and assess crisis situations to provide clinical counseling, diagnosis, brief therapeutic interventions, and necessary resources or referrals. This role also includes providing individual and family treatment as indicated. The position works collaboratively with patients, their support persons, healthcare providers, insurers, community resources, and all other involved parties.

To learn about additional Intermountain benefits: Click hereMust have experience in the justice involved space i.e. working in justice system, probation and parole, substance abuse treatment experience, etc.Essential FunctionsIdentifies high risk members unable to adhere to a medical plan or members that require complex assistance and enrolls and follows these members in longitudinal care management services.Develops and implements a care plan designed to address the medical, physical, mental, emotional, spiritual, social and supportive needs of the memberFollows the applicable established model of care or care management program policies and procedures to assess, establish and maintain a plan of care which incorporates the member's individualized needs and goals within the benefit plan throughout the care management process. Ensures the plan is evidence based and consistent with goals and objectives of referral, payer contract, or established care processes.Coordinates and facilitates communication among the member/family/representative, members of the healthcare team, and other relevant parties through interdisciplinary team meetings or other coordinated communication.Member support toward goal achievement, addressing any barriers.Face to face outreach for plan members by a member of their treatment team as required by the program model of care.Minimum QualificationsMaster of Social Work (MSW) from an accredited institution (degree verification required) or BSN.Current state licensure as an LCSW as applicable, is obtained prior to or upon completion of required supervision hours or RN.Basic computer proficiency, including familiarity with word processing and spreadsheet software.Strong written and verbal communication skills.Demonstrated ability to apply critical thinking skills.Preferred QualificationsCase Management Certification.Experience in clinical care management, social work, or working with third-party payers.Demonstrated understanding of care management principles and practices.Demonstrated understanding of health insurance products and related processes.Demonstrated understanding of coding, episode of care, and length of stay guidelines.Ability to work independently, demonstrate self-motivation, maintain a positive attitude, and adapt to a rapidly changing environment.Physical Requirements:Physical RequirementsOngoing need for employees to see and read information, documents, monitors, identify equipment and supplies, and be able to assess customer needs.Frequent interactions with colleagues and providers require employees to verbally communicate and hear and understand spoken information, needs, and issues quickly and accurately.Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy.
