Job Duties: Operate transit vehicles safely and smoothly to transport passengers. D&R operators: According to the schedule of the Mobile Data Terminal (MDT) or printed manifest. Assure compliance to all applicable DOT laws and regulations. Assists in emergency evacuations as assigned and performs other related work as required. Assure cleanliness of vehicles. Follow instructions from Dispatch and supervisory personnel and communicate any problems to dispatch. Complete daily vehicle pre/post trip inspections. Conducts operational vehicle inspections Maintain good customer/employee/supervisor relations. Report for duty promptly in proper uniform and physical condition and be ready to work any route assigned. Provide information to passengers in a helpful, courteous, efficient manner; use proper etiquette to assist passengers with disabilities. Prepare and complete all required logs and reports including reports on traffic accidents and customer incidents. Utilize the communication systems (cell phones, radios and MDT’s) in accordance with BTD policies. Use correct safety procedures to operate lift, secure wheelchairs, scooters and other assistive devices. Immediately notify dispatch of cancellations, no shows, or for directions when having difficulty locating a client. Maintain the standards of BTD, including but not limited to the Drug & Alcohol, ADA, EEO, Title VI, Code of Conduct and safety policies and procedures. The Nurse Practitioner – Advanced Illness Management AIM) CPOM facilitates advanced illness management and palliative care for our patients, ensuring a comprehensive, individualized treatment plan, while demonstrating excellent clinical skills and ethical, relevant judgment. Provides diagnosis, treatment, consultation, and follow-up under the direction of the Professional Services Corporation Physician.As a Nurse Practitioner, you will: Work in collaboration with the Professional Services Corporation (PSC) Physician or supervising physician, Primary Care Physician, and other palliative team members to deliver advanced illness management and palliative care by providing diagnosis, treatment and follow-up for patients referred to us.Take responsibility for advanced illness management from the initial assessment through discharge. Assessment at a minimum should include: disease-specific changes, decline or change in function, nutrition and/or cognition, inadequately controlled pain or other distressing symptoms, falls, infections or similar events, the impact of the disease burden on the patient's condition, caregiver and/or family's quality of life.Establish the plan of care, individualized for each patient, in consultation with the patient, family and other members of the Advanced Illness team, to include the PCP.Facilitate obtaining care for patient when a need is identified in accordance with the Plan of Care.Participates in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.Prescribe medications including controlled substances to the extent delegated and licensed, in accordance with state law. Order and interpret laboratory, imaging and other diagnostic tests within the scope of professional practice. The clinic is in an upcoming area of town where many new family homes are currently being built. You would be the only physician in clinic paired with 2 solid advanced practice providers. This is a true Family Medicine practice which provides services to all ages of family members. Approximately 5% pediatrics. Mixture of commercial insurances and Medicare. Lab & X-ray onsite with the ability to send labs to lab corps if needed.
