Being physically present at the worksite is essential for effective collaboration, communication, and supervision. You'll coordinate with third-party payers, government programs, and community services to arrange appropriate care, housing, transportation, and other essential services. Your role will involve coordinating placement and support services, including long-term care, rehabilitation, behavioral/mental health, home care, transportation, and DME, as well as assisting with CPS, APS, adoptions, and end-of-life planning, including advance directives. Additionally, you'll assist with advanced directives, end-of-life planning, and work closely with social services.
