Assist physicians in the health maintenance of patients in a complex disease state. Receive and screen all telephone requests for assistance by the existing patient base. Interview and assess patients for medical interventions in the office and on the telephone. Initiate medical interventions based on a symptom-focused protocol or guideline. Educate and monitor patient compliance regarding medical interventions. Perform interviews for the physician regarding current status of care by other providers. Perform closing interviews for the physician once the care plan has been determined. Monitor the psychosocial and emotional needs of the oncology patient for outside referrals and additional care.
