Responsibilities Include:: Care Coordination & Case Management: Partner with the clinical team to offer case management and care coordination for clients and families, ensuring seamless access to community resources and services.Community & Family Support: Assist in connecting clients and families with relevant community-based programs, educational resources, and supports from insurance or government programs, including but not limited to flex funds, transportation to care, and SNAP benefits.Referral Management: Send referrals to community programs and resources, as well as help facilitate collaboration with outpatient providers and wraparound teams.Transitions Between Care Levels: Help facilitate smooth transitions between various levels of care, such as from PRTS to community-based services, ensuring continuity of care.Prepare Clients & Families for Transition: Provide necessary information and emotional support to both the client and family to prepare for these transitions.Ongoing Resource Connection: Ensure that clients continue to receive the care they need after transitioning by connecting them with appropriate resources and services in their new care setting, following up post discharge from a Monte Nido Program.Perform other duties as assigned.
