JOB SUMMARY/PURPOSEProvides direct assessment, diagnosis, formulation, treatment planning, treatment intervention and education to patients and/or patient's family members within a clinic setting based on the Patient Centered Medical Home (PCMH) model that supports the elimination of barriers to access, including psychosocial problems.DEPARTMENT DESCRIPTIONThe Hematology & Oncology teams at Samaritan Health Services specialize in diagnosing and treating adult cancer patients using chemotherapy, hormonal therapy and biological therapy as well as treating a range of hematologic conditions. We educate patients about their illness, the treatment options and provide supportive care during treatment and into survivorship.EXPERIENCE/EDUCATION/QUALIFICATIONSLicensed Clinical Social Worker (LCSW) in State of Oregon required.Experience and/or training in Cognitive Behavioral Therapy (CBT) techniques for depression, trauma and anxiety required.Experience as a social worker in a healthcare setting preferred.Bilingual (Spanish) preferred.KNOWLEDGE/SKILLS/ABILITIESTeam Building - Ability to work, function and communicate on a multi-disciplinary team.
