The pharmacist will work at the top of their license under a broad collaborative practice agreement to play a crucial role in providing comprehensive medication management services to our complex autoimmune patient populations. This specialized pharmacist will focus on optimizing medication therapy to improve patient outcomes, reduce hospital readmissions, ensure patient access to medications, and enhance the quality of life for our patients through autonomous face-to-face and telehealth visits with patients. This pharmacist will collaborate closely with other healthcare professionals to ensure comprehensive and collaborative care as well as work on process improvement opportunities. They also will be integrated into our well-established ambulatory pharmacy team so they can collaborate with other ambulatory clinical pharmacist specialists in other markets and be part of the pharmacy team at SSM Health. The role will be actively engaged in all complexities of patient medication management, monitoring, and education, as needed. Participates in the training and education of students, residents, and physicians as applicable.Job Responsibilities and Requirements:PRIMARY RESPONSIBILITIESGathers necessary data to promote patient safety by ensuring that each relevant medication is indicated, effective, safe, and that the patient is adherent to therapyImproves patient-specific drug therapy and therapeutic problems by evaluating the appropriateness of drug therapy, assessing effectiveness, and identifying potential interactions or adverse effects.Assesses relevant patient-specific medical problems, prioritizes medical problems based on urgency and severity, and identifies preventive and health maintenance issues as needed.
