This long-term care program provides the entire continuum of medical care and support services to seniors 55 and olderwith chronic care needs, allowing them to maintain their independence in the comfort of their home without compromising their health or safety.RESPONSIBILITIESPatient Care. Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care;Communication. Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problemsEvaluation & Documentation.
