This long-term care program provides the entire continuum of medical care and support services to seniors 55 and older with chronic care needs, allowing them to maintain their independence in the comfort of their home without compromising their health or safety.RESPONSIBILITIES• Helps to provide a comprehensive plan to meet patient/participant needs, daily as well as long term.• Maintains records of patient/participant daily activities, including general condition of client.• Aids in patient/participant daily routine, activities include but are not limited to, personal care, light housekeeping, and meal preparations. • Accurately and timely completes aide activity sheets, submitting activity sheets on a timely basis as specified by department policies.• Attends a minimum of 12 hours of annual in-service education/competencies• Communicates any concerns/changes in patient/participant condition to appropriate staff in a timely manner and documents on Aide Activity sheet. • Keeps family and staff informed of clients activities and concerns.Performs other duties as requireREQUIRED QUALIFICATIONS:Nursing student who has documented evidence of successful completion of course work requiring mastery of home health aide tasks within the past 24 months.
