The selected candidate will primarily be responsible for the care and management of the patients enrolled in a partnered Acadian Health Program. Responsibilities include but are not limited to environmental assessments, comprehensive clinical assessments, interaction and treatment of patients, including facilitating continuity of care by interacting with other members of their interdisciplinary care team. Selected candidates will be responsible for participating in medical research and treatment modalities as directed by the Medical Director; as well as other operational support functions as assigned. Additional duties may include leadership roles and training/mentoring as assigned. RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO: Conduct comprehensive physical/environmental assessments based on patient-specific needsProvide non-emergent acute clinical care in a non-traditional settingWork as an extension of virtual care teams, assisting with patient care and coordination under personalized-patient medical directionSupport and monitor the patient's care plan, identifying and reporting any deviations from the prescribed patient care planEngage with patients and their care teams through virtual conferencing and in-person home visitsProvide disease-specific education to the patient and/or caregiversCollaborate with various agencies and Acadian Health team members to maintain seamless continuity of care, including coordination with physicians, nurse practitioners, physician assistants, pharmacists, and hospital case managersMaintain timely and accurate patient medical records utilizing electronic health record softwareMake informed decisions in a dynamic and fast-paced work environmentParticipate in community health initiatives such as blood pressure or immunization clinics, to promote Acadian CompaniesPerform other duties as assigned by the Acadian Health Operations Director or Medical DirectorQUALIFICATIONS Hold an Associate Degree in Nursing; Bachelors preferred Hold an active Registered Nursing license in the state of employment Current BLS and ACLS certifications established by the American Heart Association Current and valid state-issued driver's license determined by appropriate state requirements and acceptable driving record in accordance with Acadian's driving and insurability policiesEffective verbal, written, and interpersonal communication skillsAbility to provide effective coaching and leadershipAbility to adhere to established stands for educational qualityFamiliarity with basic computer applications and functionsTeam-oriented and able to work effectively and efficiently with othersAbility to lift 50 lbs safelyTRAINING REQUIREMENTS Orientation to job requirementsCompletion of the Acadian Health clinical education programCompletion of ride-along shifts with current Acadian Health StaffClearance to perform; issued by an Acadian Health Operations SupervisorParticipation in the Acadian Health Continuing Education programAll qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, disability, or status as a protected veteran.
