Initial assessment on consultation Weekly & PRN assessment of treatment side effects throughout course of treatment, i.e. nausea, diarrhea, xerostomia, skin in treatment portal. Notes abnormal lab results and brings to MD or NP attention. Assess current nutritional status (weekly weight, nutritional counseling as needed, dietitian referral as needed).Initiates/develops multidisciplinary care plans and discharge plans in accordance with department standards and using appropriate tools.Sets goals and formulates appropriate multidisciplinary interventions to meet these goals.Integrates psychosocial, physical, nutritional, and rehabilitation needs of the patient in-patient care and discharge planning. Works with patient and family in allaying concerns related to radiation therapy. Assist patient in coping and maintaining their support system. Utilizes available resources to assist the patient/family as needed, i.e. Social Worker, UNA, Chaplain, Psych, Clinician, Transportation, Business Office. Remains accessible to patient and family members realizing their need for frequent reassurance and explanation.Performs interventions in accordance with current policy and procedures, safety guidelines and infection control principles.Provides patient/family education according to the plan of care and documents same. Assess patient/significant others learning needs and provide information based on those needs. Participates in pre-radiation / pre-procedure preparation & education of patients / family. Utilizes appropriate educational literature for individual patients. Participates in development & evaluation of patient education materials.Delegates and prioritizes work assignments according to the individual associate’s competence and patient care needs.Evaluates effectiveness, interventions and revises plan of care accordingly.Transmits pertinent patient information to appropriate individual (i.e., MD, NP, supervisor, discharge planner, etc.)Complies with unit and departmental scheduling guidelines.Implements and maintains IV therapy on patients according to Hospital policy and standards of care.Manages the care of the pre-operative/pre-procedural patient according to Hospital policies and standards of care.Demonstrates competency in pain management by recording the patient’s pain intensity, implementing and evaluating interventions based on the plan of care and providing patient/family education.Gives input to Director/Manager regarding peer evaluations.Gives input to Director/Manager regarding unit recruitment and retention issues.Serves as a preceptor to new staff hires.Ensures a safe environment in aspects of care, equipment and treatment.Identifies and reports potential and actual patient safety issues.Correctly complies with all national patient safety goals/ risk reduction procedures and associated policies as applicable to scope of practice.Assists as directed in PI activities including data collection and analysis.Proficient in medication administration, including use of 2 patient identifiers.Proficient in the use of the Fall Prevention Protocol.Performs ongoing evaluation of patient’s status and physical and psychological response to radiation. Evaluates effectiveness of medications.Collaborates with technicians, doctors and other departments in scheduling radiation planning procedures, tests, operating room, etc.Other Duties:This role works collaboratively with RNs, Physicians, Nurse Practitioners, and Medical Assistants.
