We’ve been involved in work groups for our hospital readmissions, palliative care, advance care planning, and pneumonia quality groups, etc. The Disease management therapists also see hospital outpatients for tobacco dependence counseling and asthma education, using a clinic room at the Broadway campus. - This position is Full time 8 hour day shifts.
