The Manager - Clinical Operations participates in all activities related to the professional services provided, including establishing criteria on qualifications and assignment of clinical personnel. As a Manager - Clinical Operations you will:Key Responsibilities:Monitor to ensure coordination and follow-through is complete for all referrals to the hospice programAssist in the coordination and assignment of all newly admitted hospice patients to the appropriate team and/or disciplinesWork collaboratively to ensure all admission documentation is completed within 24-hour period and dispersed among team membersEnsure communication of all patient admissions and changes in Plan of Care is appropriately relayed to each member of the Interdisciplinary Team and Primary Care PhysicianServes as member of IDT committee and participates in all IDT meetings in which admissions, certifications, recertification’s and medical management of the patients are discussedConsults with the Hospice Medical Director in matters relating to patient care servicesParticipating in routine visits and admissions of patients in times of high census or low staff availabilitySelects and maintains a qualified, well-organized staff to provide care for the needs of the patientsIs accountable and supervises the IDT team i.e. RN's, LPN's, Medical Social Workers, Chaplin’s, and Hospice AidesEstablishes staffing patterns, which reflect the quality and quantity of various professional personnel necessary to plan, provide, and supervise the care rendered to patients and familiesEstablishes methods for coordination of care by all disciplinesBe responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly CareSkills and Experience Required:Is currently a registered nurse licensed in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)Two years or more of supervisory and/or administrative experience managing clinical teams5 or more years of nursing experienceMust maintain a valid driver’s license and good driving recordDemonstrates a broad base knowledge of community health nursing and competent clinical skillsAbility to be available in person or telephonically to assist with after-hours administrative on call responsibilitiesProficient Computer skills including, but not limited to, use of Word, Excel, report software such as HomeCare HomeBase, and OutlookExcellent verbal and written communication skills with demonstrated leadership abilityExcellent organizational skillsAbility to travel (up to 25% or as needed)Preferred Knowledge, Skills or Experience:Certified Hospice and Palliative NursingPrevious experience with Electronic Medical RecordsHospice experiencePrevious experience in handling new hospice referrals from first contact through admissionA working knowledge of Community Health Accreditation Program (CHAP) on Accreditation, Quality Assurance, and Hospice Knowledge
