This patient primarily treats Adult / Adolescent Ortho patients, however, also has Men's/ Women's Health, Lymphedema/ Oncology patients. About the unitThis unit is a 16-bed cardiovascular/cardiothoracic care unit. This unit’s primary specialty is post-open heart/CABG patients. Post-op CABG patients will recover in the ICU for 2-3 days. The patient population will include cardiac procedure requiring ICU level care, open-heart patients direct from the OR, vascular surgery, thoracic surgery, TAVR procedure, and STEMI intervention. RNs will care for patients requiring ventilator support, SWAN-GANZ catheters, vasopressors, inotrope drips, high alert drips, intra-aortic balloon pumps, continuous renal replacement therapy (CRRT), IMPELLA devices, and chest tubes. About the unit10N is an inpatient unit specializing in the treatment of patients with hematological and oncological illness with the additional capacity to care for 12 cardiac telemetry patients. Working with oncology patients will provide you with an incredible amount of learning opportunities including working with chemotherapy, learning about radiation therapy, end of life care, and assisting with some unique bedside procedures to name a few. About the unit6 North is a 32-bed surgical floor that offers a great learning environment fostered by our team of exceptional nurses. 6 North offers experiences that are normally not found in Med-Surg clinical rotation. The unit has a focus on surgical conditions including urology procedures, major and minor abdominal surgeries, ENT, bariatric and pulmonary surgeries, peripheral vascular procedures as well as other general surgical post-op needs. About the UnitA Wing is a 58 bed Medicare/Medicaid unit that has residents with multiple clinical issues including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac related issues needing monitoring. Nurses work closely with a variety of health care professionals including: doctors, social work, dieticians, PT/OT, hospice, and spiritual support. A Wing offers a well-rounded opportunity for learning, professional growth and personal satisfaction.B Wing is a 40 bed Medicare/Medicaid/Dementia unit. We provide care to residents with a variety of diagnoses. B wing is a great unit for nursing students to develop their organizational skills and gain some experience in dealing with challenging behaviors. This unit also works closely with a multidisciplinary team meets and discusses the resident’s plan of care.C Wing is a 58 bed Medicare/Rehabilitation and Skilled unit and is a great place to learn basic med-surg, I.V. therapy & wound treatments nursing skills. This fast paced and busy unit provide a great atmosphere for PT/OT residents to be motivated and reach their goals.About the facilityWith 2 floors, 4 units, we are able to provide care for long term care residents and post-acute rehab patients. The first floor has 96 beds and the second floor has 100 beds. Nurse-patient ratios vary depending on the shift and the population you will be serving. We care for dementia residents, long term care and a rehab residents. With 196 beds, we accept but not limited to patient specialties, such as general rehab, neuro rehab, cardiac, respiratory and cancer. Scope of workThe hospital based Wound Clinician serves as a clinical resource and consultant for the acute care of patients with disorders of the gastrointestinal, genitourinary, and integumentary systems. The care extends to those with acute and chronic wounds (for example, complex draining wounds, fistulae, pressure injury, vascular ulcers), ostomies, percutaneous tubes, and continence disorders. Professional responsibilities are carried out through direct patient care, clinical consultation, education, research, and administration.General duties/responsibilities· Utilizes the nursing process when directing or delivering care to patients with WOC needs including assessment, diagnosis, outcomes identification, planning, and implementation (e.g., coordination of care delivery, health teaching and promotion, consultation), and evaluation.· Utilizes and maintains current physical, psychosocial, clinical assessment, and therapeutic communication skills to determine individual patient needs and expected outcomes.· Participates in committees or task forces related to wound, ostomy, and continence issues.Wound/skin care· Provides consultation for individuals with altered skin integrity (e.g., pressure injuries, draining wounds, fistulae) to develop individualized plans of care and attain expected outcomes.· Provides guidance to staff in implementing protocols to identify, control, or eliminate etiologic factors for skin breakdown, including selection of appropriate support surfaces.· Provides appropriate education to patients, caregivers, and staff regarding skin care, wound management, care of percutaneous tubes, and draining wound/fistulae management.· Provides consultation for negative pressure wound therapy, (including subsequent dressing changes) provides collaborative care which includes patient’s next care setting.· Validates pressure injury data collection for nursing quality indicators.WOC cares by providing the following:· Formal or informal continuing education programs.· Orientation sessions.· Collaborative practice committees.· Nursing councils.· Attends continuing education programs related to WOC nursing to maintain current knowledge and skills.QualificationsRequired Bachelor's Degree BSN2 years of relevant experience Two to four years of recent, relevant nursing experience. This position offers up to a $5000.00 starting bonus!Post Acute Care UnitsA Wing is a 58 bed Medicare/Medicaid unit that has residents with multiple clinical issues including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac related issues needing monitoring. Nurses work closely with a variety of health care professionals including: doctors, social work, dieticians, PT/OT, hospice, and spiritual support. A Wing offers a well-rounded opportunity for learning, professional growth and personal satisfaction.B Wing is a 40 bed Medicare/Medicaid/Dementia unit. We provide care to residents with a variety of diagnoses. B wing is a great unit for nursing students to develop their organizational skills and gain some experience in dealing with challenging behaviors. This unit also works closely with a multidisciplinary team meets and discusses the resident’s plan of care.C Wing is a 58 bed Medicare/Rehabilitation and Skilled unit and is a great place to learn basic med-surg, I.V. therapy & wound treatments nursing skills. Nurses on this unit along with host of healthcare professionals work to provide and implement the best treatment plan for residents. This fast paced and busy unit provide a great atmosphere for PT/OT residents to be motivated and reach their goals.About the facilityWith 2 floors, 4 units, we are able to provide care for long term care residents and post-acute rehab patients. The first floor has 96 beds and the second floor has 100 beds. Nurse-patient ratios vary depending on the shift and the population you will be serving. We care for dementia residents, long term care and a rehab residents. With 196 beds, we accept but not limited to patient specialties, such as general rehab, neuro rehab, cardiac, respiratory and cancer. We also have various programs for residents to participate inmultiple activities.Scope of workLicensed Practical Nurse (LPN)Under the direction of the RN/Mid-level Provider/Physician and in collaboration with other members of the health care team, contributes to the assessment, planning, implementation, and evaluation of patient care. Informs patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate, in coordination with the RN/Mid-level Provider/Physician. Documents care provided via computer/written patient record in a manner consistent with guidelines.QualificationsRequired Graduate of an approved LPN program.Experience in Skilled Nursing/Long Term Care.
