About the UnitA Wing is a 58 bed Medicare/Medicaid unit that has residents with multiple clinical issues including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac related issues needing monitoring. Nurses work closely with a variety of health care professionals including: doctors, social work, dieticians, PT/OT, hospice, and spiritual support. A Wing offers a well-rounded opportunity for learning, professional growth and personal satisfaction.B Wing is a 40 bed Medicare/Medicaid/Dementia unit. We provide care to residents with a variety of diagnoses. B wing is a great unit for nursing students to develop their organizational skills and gain some experience in dealing with challenging behaviors. This unit also works closely with a multidisciplinary team meets and discusses the resident’s plan of care.C Wing is a 58 bed Medicare/Rehabilitation and Skilled unit and is a great place to learn basic med-surg, I.V. therapy & wound treatments nursing skills. Nurses on this unit along with host of healthcare professionals work to provide and implement the best treatment plan for residents. This fast paced and busy unit provide a great atmosphere for PT/OT residents to be motivated and reach their goals.About the facilityWith 2 floors, 4 units, we are able to provide care for long term care residents and post-acute rehab patients. The first floor has 96 beds and the second floor has 100 beds. Nurse-patient ratios vary depending on the shift and the population you will be serving. We care for dementia residents, long term care and a rehab residents. With 196 beds, we accept but not limited to patient specialties, such as general rehab, neuro rehab, cardiac, respiratory and cancer. Services include but are not limited to application of therapeutic techniques in areas such as:* Therapeutic exercise including aerobic capacity conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training* Functional training* Manual therapy* Airway clearance techniques* Integumentary repair and protection* Use of electrotherapeutic, physical agent and mechanical modalities* Health and Wellness* Education on Chronic Disease Management * Falls Assessment and Interventions to reduce fall risk* Case Management in the home and community12. Designs, fabricates, fits and/or applies adaptive devices, and trains patients in the use of adaptive devices included orthotics and prosthetics.13. Selects appropriate activities suited to the individual's physical and mental capacity, intelligence level and interest.14. Instructs patient in the skills and techniques required for participation in mobility activities and evaluates patient progress, attitudes, and behavior as related to rehabilitative potential.15. Performs routine reexamination as needed/required to modify/progress plan of treatment.16. Incorporates health and wellness programming into treatment plans for patients.17. Performs home and community mobility assessments and provides recommendations for safe mobility in the home and community space.18. Performs other related duties as required. Qualifications: 1. Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility; and2. They must have a Master's degree in Physical Therapy; or3. They must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy; or4. They must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience.5. The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy.6. Must have a valid driver's license or reliable, independent transportation to and from the patients' residence.. Posted Salary Range: USD $60.00 - USD $60.00 /Hr.
