About the unitBig Rapids Campus has a 22 bed medical surgical unit and a 4 bed critical care unit. This campus is the perfect place to gain experience as a new nurse and grow your career! The patient population in Big Rapids is very diverse, it consists of primarily hospitalist medicine patients with a wide variety of diagnosis, some of which include: pneumonia, COPD, cellulitis, kidney failure, and bowel obstructions. These diagnoses provide the opportunity to develop acute care skills such as IV placement, chest tubes and NG placement/management. In our critical care unit it is common to see high acuity patients with sepsis, hyperglycemia and respiratory failure whose plan of care would include managing medication drips (insulin, vasopressors, etc.) and the utilization of equipment such as ventilators and BIPAPs. Big Rapids has a well-established team that works collaboratively with an interdisciplinary team that includes hospitalists, care managers, social workers, pharmacy, nurse techs, physical therapists, and occupational therapists to provide quality care for our patients. We have a good mix of experienced RNs that work alongside our education team to provide our new hires and grads with a great onboarding experience. As a RN Case Manager you will:Under the physician’s order, admit patients eligible for hospice servicesAssess and evaluates patient needs/problems, identifies mutually agreed upon goals with patientsReport patient status and need for other disciplines to clinical leadership, attending physician and hospice physicianUpdate care plans on an ongoing basis; revise and resolve patient problems and goals as changes occur and/or recertificationComplete informational visit and obtain patient consents for hospice admission per office procedureBe responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care QualificationsCurrent unencumbered registered nurse in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)Must maintain a valid driver’s license and good driving recordAbility to work in a field setting and exhibited ability to make sound nursing judgmentsAbility to assess patient needs and formulate individualized patient care plans to meet those needs Pay TransparencyIndividual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.
