As an On-call Registered Nurse you will:Key Responsibilities:Assume primary responsibility for the patient/family caseload that includes assessing, planning, implementing, and evaluating care needs during on-call hours in collaboration with the triage teamReport On-Call activities to the clinical leadership and Branch RN Case Managers at the end of the call shiftMaintain and complete appropriate care coordination notes for visit notes, and other pertinent clinical informationAssist, provide, or arrange for delivery of medications and supplies needed after business hours, weekends, and/or holidaysAs scheduled, attend interdisciplinary team meetings during on-call period for full team discussion, consultation and evaluation of patient/family needsUnder the physician’s order, admits patients eligible for hospice services in collaboration with the triage teamReports patient status and need for other disciplines to clinical leadership, attending physician and hospice physicianDevelops patient care plan that specifically addresses identified patient problems; nursing problems and goals that has been identified as mutually agreed upon goals with patient(s) during on-call hours, weekends and/or holiday(s)Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly CareSkills and Experience Required:Is currently licensed in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)Current driver’s license and automobile in good working condition with proof of auto insuranceTwo years recent experience as a professional nurseIs able to meet certain physical demands of standing, bending, lifting, stooping or performing other work requiring light physical exertion (up to 30 pounds) on a continuous basis (over 50% of time); or moderate physical exertion (30 to 50 pounds) on a frequent basis (16 to 50% of time); or heavy physical exertion on an occasional basis (up to 15% of time)Ability to work in a field setting and exhibited ability to make sound nursing judgments; must be able to work independentlyAbility to assess patient needs and formulate individualized patient care plans to meet those needsKnowledge of physical, psychosocial and spiritual needs of terminally ill patients and their caregiversEffective written and verbal communication skillsPreferred Knowledge, Skills and Experience:Two years’ experience within hospiceCertified Hospice and Palliative NursingElectronic Medical Records Experience
