The ACT RN is a whose primary responsibilities include: at the guidance of the ACT team leader, the ACT Team Member functions as a member of the multidisciplinary team rather than an individual practitioner. An ACT mental health professional assesses, plans, develops, coordinates, and directly provides flexible treatment, rehabilitation and support services to individuals who have the most serious symptoms of mental illness and who consequently have the greatest difficulty with basic daily activities such as keeping themselves safe, caring for their basic physical needs, maintaining a safe and affordable place to live, maintaining sobriety, and other activities of daily living. Rather than brokering services, the team itself is the service delivery vehicle, and assumes direct responsibility for providing the specific mix of medically necessary services required by the persons-served, if there is a need.
