Oversees person served individualized recovery process relating to their health through support in managing their disease through clinical treatment.Embraces the concept of Recovery by providing a stable and safe environment and supporting the engagement of meaningful activities.Documents all encounters and contacts made on behalf of persons served, within designated timeframes.Provides and/or assists in providing appropriate medical, nursing, or other health care in various settings, including community-based settings, as neededParticipates in the assessment of the physical, functional, and psychological needs of persons served, as required.Participates in the development of Treatment Plans by assisting persons served in setting goals linked to addressing identified medical and psychiatric needs, as required.Provides referral to appropriate community resources, facilitate access and communication when multiple services are involved; monitor activities to ensure that services are being delivered and meet the needs of the persons served; coordinate services to avoid duplication.Maintains accurate records and confidentiality pertaining to persons served.Monitors persons served progress toward health care related goal achievements and periodically reassess changes in methods and techniques; coordinate other services as needed.Conducts health screenings and perform health assessments as necessary.Takes the lead role and participates in providing medication administrative management and monitoring.Attends and actively participates in program’s staff team meetings.
