OverviewThe Community Health Initiatives team works closely with other health system leaders, cross-sector community partners, and community members to identify, develop, and implement proven strategies and tactics to address complex obstacles to child and family health, educational attainment, and economic self-sufficiency. The team partners with community residents, leaders, public and private sectors, and local organizations to leverage community resources and build neighborhood capacity that will help transform disparate neighborhoods/regions into places of opportunity. The nurse supervisor works in conjunction with the Home Visiting Services Manager to support the daily operational effectiveness of an integrated, evidence-based home visiting program for low-income families, focusing on prenatal, early childhood, and family health. Through ongoing home visits families receive the care and support they need to have a healthy pregnancy, provide responsible and competent care for their children, and become more economically self-sufficient. The nurse supervisor will also assist in the oversight, staff development, and supervision of Community Health Initiatives new home visiting program Family Foundations. Other key responsibilities include staffing and professional development of team members, adherence to standards of professional practice, the provision of client-centered care and services, home visiting model fidelity, oversight of quality improvement and evaluation activities, and the development of community integration processes/workflows (outreach, documentation systems and shared care planning, support and referral systems, and others as identified). QualificationsCertifications: IL RN license, Certified Lactation Counselor (CLC) with in 2yrs, Drivers license, Proof of Auto Insurance, Education: Bachelors in Nursing , Work Experience: ResponsibilitiesAssists with recruitment, hiring, development, evaluation, and retention activities.Develops and oversees the orientation, continuing education, and competencies for new and current staff.Works with the Home Visiting Services Manager on defined financial goals and ensures that team is fiscally responsible.Ensures adherence to established standards of care and scopes of practice.Assesses and develops tactics to address knowledge and skill gaps of staff. Provides professional development opportunities to remediate gaps.Develops and implements external and internal systems to assure timely receipt and disposition of referrals to and from the program.Coordinates daily operations, assigns client caseloads, and monitors nurse home visitor's ability to build and manage family panels; effectively manages workflows and systems to enable home visitors to focus on clients.Supervises a culture of caring, trust, as well as support and growth with families, community members, and team members.Provides one-to-one clinical supervisions, case conferences, team meetings, and field supervision per NFP requirements.Integrates principles and concepts of motivational interviewing, coaching and reflective practice into operations (modeling therapeutic relationships, change theory, self-efficacy, client centered, strength based, and solution focused approachMaintain a small caseload of Healthy Beginnings FamiliesParticipates in review and analysis of reports for achievements and areas for improvement as well as quality improvement efforts. Compensation and BenefitsThe compensation range for this position is $41per hour - $70.52per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health.
