The Primary Care Patient Care LPN will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for all patients. Duties and responsibilities will be performed in a manner consistent with our mission, values, and Mercy Service Standards.Job Description:Responsibilities• Responsible for coordinating care and building relationships with patients in partnership with Mercy Primary Care Providers.• Cleans and stocks examination rooms including laying out special equipment or supplies to be used during a patient visit.• Escorts patient to the examination room and prepares patient for examination and treatment in a respectful and considerate manner.• Prepares patient for visit with provider by completing Advanced Standard room process including entering the chief complaint; taking patient vital signs; reviewing patient history and allergies; completing a thorough medication reconciliation; working the Encounter Guide including helping to close any open care gaps such as fall risk, depression screening, breast cancer screening, colorectal cancer screening, and diabetic eye exams; rechecking elevated blood pressures; and completing needed documentation in Epic for yearly preventative visits as needed.• Assists with any needed in-office testing including, but not limited to, drawing blood, completing mental status exams, EKGs, walking oxygen studies and point of care testing.• Administer vaccines and other injections within scope of practice.• Assists provider with procedures.• Inputs orders as needed to aid provider and clinical care team.• Schedules follow up appointments when needed per office policy.• Ensures excellent consumer experience.• Collaborates with providers and clinical care team to monitor and coordinate delivery of the care plan.• Performs other work duties as assigned.Qualifications:Minimum Requirements:Education: Graduate of an accredited practical nursing program.Licensure: Licensed as an LPN (Licensed Practical Nurse) in the state of practice.Preferred Experience: 6 months clinical healthcare experience, preferably in a primary care office.Other Knowledge, Skills, and Abilities:· Works well as a member of a team and willing to collaborate with all members of the care team· Excellent written and oral communication skills· Customer service orientation· Excellent organizational and interpersonal skills· Flexible and positive attitude· Attention to detail and accuracy· Self-directed and able to perform tasks that are in scope of practice independently with minimal oversightPhysical Requirements:Position requires individuals to push, pull, and/or lift 50 pounds on a regular basis.Position requires prolonged standing and walking each shift.Position requires the ability to grip, reach, bend, kneel, twist, and squat to perform duties.We Offer Great Benefits:Day-one comprehensive health, vision and dental coverage, PTO, tuition reimbursement and employer-matched retirement funds are just a few of the great benefits offered to eligible co-workers, including those working 32 hours or more per pay period!We’re bringing to life a healing ministry through compassionate care.At Mercy, our supportive community will be behind you every step of your day, especially the tough ones.
