Job Description Department Description The APP is responsible for management of outpatient kidney transplant patients with the multidisciplinary team. Provides direct patient care, evaluation and medical management, development of assessment and plans for patients in outpatient kidney transplant clinic. Completes the documentation and serves as the primary provider for patients seen via telemedicine, in person clinic, and outreach clinics. Receives phone calls from patients and patient families and triages and escalates patient care alongside the physician staff. Participates in discharge planning and care coordination for transition of patient from inpatient to outpatient care. Completes lab review, medication management, immunosuppression management, and medical assessment of outpatient kidney transplant patients in person or over the phone. The APP is responsible for continuity of care and interfacing with other service lines for consultation when appropriate.The APP is responsible for assisting in transition of care from outpatient to inpatient hospital admissions for kidney transplant patients and working with the inpatient Transplant Nephrology team to transition patient care from inpatient to outpatient. The APP works with other members of the multidisciplinary team including Physicians, Fellow, Residents, Social Work, Case Management, Nursing Staff, Pharmacy, Dietician, and other service line providers.
