The RN will contribute to developing, reviewing, and revising all diabetes education material. Provides evidence based, clinical best practice in diabetes education to persons with diabetes and their family members or caregivers. Participates as a member of interdisciplinary healthcare team to facilitate the continuum of care and education of patients with diabetes. Serves as a consultant/resource to the clinical areas, clinic staff, and departments as it relates to diabetes education. Cultivates positive and collaborative relationships with nursing and clinical leadership teams to ensure that educational needs are being met in the most effective manner possible. Provides leadership through educational activities to bring about changes in clinical practice regarding diabetes care and education. Evaluates diabetes education programs on a regular basis and plans and implements improvements based on this evaluation and feedback from key stakeholders. Monitors and evaluates, as needed, resource utilization, patient outcomes, patient's skills in self-care, and patient's ability and readiness to learn. Ensures correct and timely documentation in the patient's medical record following individual consultation. Counsels and educates individual patients appropriately on diabetes care in conjunction with patient's medical provider (diabetes medications, exercise, diet, sick days, etc.). Participates in root cause analysis on hypoglycemia and hyperglycemia events to identify possible causes and when appropriate, make recommendations to medical staff and/or pharmacy staff to consider diabetes medication adjustments. Utilizes adult learning principles in program planning to select the teaching strategy and method appropriate for the learner and the content including e-learning. Develops and conducts group presentations and related educational programs, and staff in services, as needed. When developing diabetes education for clinical staff, the curriculum incorporates MLKCH clinical Policies and Procedures and recommendations from Quality and Performance Improvement activities. Uses, protects, and discloses MLKCH patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards. As a Substitute Instructional Aide, you will provide individualized supervision and instruction to assigned students and additional support to the core classroom staff. In the absence of the regular classroom aide, you will work with student(s) to implement their individualized education programs (IEP) through use of group and individual instruction in both the classroom and community. Responsibilities Include: Working with students to improve their community, vocational, leisure, domestic, academic, and social skills.Implementing curriculum (including all on-site and community-based instruction) and behavioral intervention strategies consistent with the letter and intent of the IEP/ITP (Individual Transition Plan)/PBIP (Positive Behavior Intervention Plan) to meet each student’s goals and objectives.Instructing and monitoring students during lessons in the classroom to ensure fidelity of instruction.Encouraging and monitoring the progress of individual students in accordance with specific timelines and communicating with the classroom teacher regarding overall progress and/or interventions needed.Completing accurate and timely documentation on student’s behavior as outlined by school procedures; reporting observations to relevant staff and making recommendations based on observations.Assisting in preparation of other behavioral reports and daily activities log as required.Maintaining confidentiality of student information and records at all times.Participate in school meetings, student/parent meetings and individual student advisory meetings as requested and required.Responding constructively to formal and informal feedback.Performing other duties as assigned. This position operates with oversight and guidance from the classroom teacher and program director in carrying out the essential duties and responsibilities to provide support to assigned students and Spectrum program site. Qualifications Required: High school diploma or equivalent w/ passage of basic skills test.Associate degree or higher in education or a closely related field of study preferred.Hold currently or have the ability to obtain a state driver's license.Licensed currently or in the process of obtaining a paraprofessional, paraeducator, teaching assistant or similar credential preferred.Prior experience working with children, preferably in a special education program setting.Highly skilled in working with children with autism spectrum disorders, developmental disabilities and/or challenging behaviors.Comfortable driving and/or riding in a company van with students before and/or after school.Ability to think and act quickly and calmly in an emergency and make independent decisions.Proficiency in providing motivation and having critical conversations with students that help move them toward realistic goals.Advanced oral and written communication, organization, conflict resolution, multitasking, problem solving, and decision-making ability.Well-versed in the use of relevant technology including experience with computers, Microsoft Office Suite, database entry and basic office equipment.
