The RN will contribute to developing, reviewing, and revising all diabetes education material. ESSENTIAL DUTIES AND RESPONSIBILITIES Matrixed work relationship to the Clinical Manager Pharmacy and takes clinical direction on standards of care for MLKCH patients with diagnosis of diabetes. Provides evidence based, clinical best practice in diabetes education to persons with diabetes and their family members or caregivers. Participates as a member of interdisciplinary healthcare team to facilitate the continuum of care and education of patients with diabetes. Serves as a consultant/resource to the clinical areas, clinic staff, and departments as it relates to diabetes education. Cultivates positive and collaborative relationships with nursing and clinical leadership teams to ensure that educational needs are being met in the most effective manner possible. Provides leadership through educational activities to bring about changes in clinical practice regarding diabetes care and education. Evaluates diabetes education programs on a regular basis and plans and implements improvements based on this evaluation and feedback from key stakeholders. Monitors and evaluates, as needed, resource utilization, patient outcomes, patient's skills in self-care, and patient's ability and readiness to learn. Ensures correct and timely documentation in the patient's medical record following individual consultation. Counsels and educates individual patients appropriately on diabetes care in conjunction with patient's medical provider (diabetes medications, exercise, diet, sick days, etc.). Participates in root cause analysis on hypoglycemia and hyperglycemia events to identify possible causes and when appropriate, make recommendations to medical staff and/or pharmacy staff to consider diabetes medication adjustments. Utilizes adult learning principles in program planning to select the teaching strategy and method appropriate for the learner and the content including e-learning. Develops and conducts group presentations and related educational programs, and staff in services, as needed. When developing diabetes education for clinical staff, the curriculum incorporates MLKCH clinical Policies and Procedures and recommendations from Quality and Performance Improvement activities. Uses, protects, and discloses MLKCH patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards. POSITION REQUIREMENTS A. Education Associates Degree in Nursing required. Bachelor Degree in Nursing preferred. B. Qualifications/Experience Minimum of two (2) years of clinical nursing experience and a minimum two (2) years diabetes education experience with inpatient and outpatient settings. Responsibilities Include: Collaborating closely with classroom teachers to cultivate a supportive, welcoming, and high-achieving learning environment where all students feel safe and valued.Implementing engaging, individualized instruction and behavioral interventions both on-site and in community settings, aligned with each student's unique goals and objectives.Instructing and closely monitoring students during classroom lessons, ensuring instructional fidelity, and applying targeted strategies to enhance student comprehension and successful course completion.Encouraging individual student growth by consistently tracking progress against established timelines and proactively communicating with classroom teachers regarding students' successes and areas needing additional intervention.Managing student behavior effectively, reinforcing our school's philosophy and policies, and applying consistent disciplinary measures in collaboration with classroom teachers.Supporting and mentoring students facing academic challenges, encouraging personal growth, and assisting them in developing essential community, vocational, leisure, domestic, academic, and social skills.Completing clear, accurate, and timely documentation of student behaviors and performance, reporting meaningful observations, and offering actionable recommendations to relevant staff.Assisting with the preparation of detailed behavioral reports, daily activity logs, and other necessary documentation.Communicating proactively and clearly with students, colleagues, supervisors, and families regarding student needs, achievements, and areas of concern, working in close partnership with teachers and administration.Maintaining strict confidentiality of all student records and information at all times.Participating actively in assurance and accreditation activities, contributing to achieving site performance standards and accreditation requirements.Participating in school-wide, student-parent, and advisory meetings to foster collaborative support networks and positive educational outcomes.Responding positively and constructively to formal and informal feedback, demonstrating continuous professional growth.Performing additional responsibilities as needed to support student success and contribute to the overall effectiveness of our educational community. Qualifications Required: High school diploma or equivalent w/ passage of basic skills test.Associate degree or higher in education or a closely related field of study preferred.Hold currently or have the ability to obtain a state driver's license.Licensed currently or in the process of obtaining a paraprofessional, paraeducator, teaching assistant or similar credential preferred.Prior experience working with children, preferably in a special education program setting.Highly skilled in working with children with autism spectrum disorders, developmental disabilities and/or challenging behaviors.Comfortable driving and/or riding in a company van with students before and/or after school.Ability to think and act quickly and calmly in an emergency and make independent decisions.Proficiency in providing motivation and having critical conversations with students that help move them toward realistic goals.Advanced oral and written communication, organization, conflict resolution, multitasking, problem solving, and decision-making ability.Knowledge of relevant technology including experience with instructional technology, Microsoft Office Suite, database entry and basic office equipment.
