Navigates patients through the program for treatment in a timely manner. Responsible for facilitating rapid scheduling and followup tracking
patients to ensure completion of their recommended treatment plan. Identify and problem solve any service gaps identified. Promote the
Navigator role and treatment awareness to the community through presentations and participation in related organizations. Staffs multi
disciplinary conference and coordinates loop closure with referring providers. Collects quality data on patients
