Job Summary:

Drives the success of care coordination programs regionally, focusing on case management, social work, care transitions, and total care progression. Provides the vision, strategic guidance, direction, and overall management to improve clinical outcomes within the assigned region. Focuses on care transformation system key performance indicators (KPIs) while acting as the operational case management liaison for region to support local teams.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Manages the operation of the case management and social work teams while providing care coordination across the care continuum with focus on system initiatives.
  • Provides strategic direction to cross-functional teams and department leaders to ensure cohesive and effective implementation of prioritized care transformation (CT) initiatives, overseeing the implementation and standardization of inpatient care delivery.
  • Ensures case management and social work teams adhere to and enforce standard work for care progression is being followed on a daily basis to ensure reduction in length of stay for patients. Manages internal and external stakeholder relationships to communicate progress, risks, issues, and mitigation to care transformation (CT). Communicates progress, risks, issues, and mitigation to leadership to proactively drive resolution.
  • Utilizes data analytics and trending to forecast future needs, programs to improve functional areas with increased patient quality outcomes and reduced costs with efficient processes.
  • Maintains an ongoing awareness of regulatory requirements to ensure compliance.
  • Maintains knowledge of best practices and industry insights related to case management for purposes of continued strategic planning and innovation. Supports CT leadership in strategic planning, innovation, goal setting and other areas.
  • Implements a management system that is customer-focused, encourages efficiency and automation, drives performance.
  • Works with clinical, product, and other teams to identify and implement innovative care management programs. Develops and coordinates strategies for ensuring delivery of care in the most appropriate, lowest cost setting environments. Assumes fiscal responsibility for budgetary planning and implementation for assigned areas.
  • Applies the existing body of evidence-based practice and scientific knowledge in health care to nursing practice, ensuring that nursing care is delivered based on patient’s age-specific needs and clinical needs as described in the department's scope of service.
  • Recruits, engages, develops, leads, and manages assigned staff.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.

EDUCATION

  • Bachelor’s Degree

EXPERIENCE

  • Seven years’ with five in leadership

REQUIRED PROFESSIONAL LICENSE AND/OR CERTIFICATIONS
