Registered Nurse – Hospice

Location:Homes throughout Carroll County, MD
Schedule:Full-Time | Monday–Friday | 8:00 AM – 4:30 PM

Position Summary:

Bridging Life Hospice isseekinga compassionate and dedicatedHospice Registered Nurse (RN)to provide high-quality, patient-centered care to individuals facing end-of-life in facility settings across Carroll County. This role offers autonomy, meaningful connections, and the opportunity to make a profound impact on patients and their families during one of life’s most critical transitions.

Key Responsibilities:

• Collect and document comprehensive health status data using patient records, family input, and available resources.

• Formulate, implement, and evaluate individualized plans of carein accordance withagency policies.

• Prioritize and coordinate care in a responsive and autonomous manner.

• Provide emotional and clinical support to patients and families during the terminal phase of illness.

• Perform rounds with physicians and communicate any care changes to the hospice team.

• Collaborate with and supervise Hospice Aides/HHAs, adjusting care plans as needed andprovidingshift-by-shift guidance.

• Participate in interdisciplinary team (IDT) meetings andsubmittimelydocumentation.

• Support performance improvement efforts by responding promptly to documentation and information requests.

• Serve as a resource and positive role model for team members andnew staff.

• Activelyparticipatein cost-effective care strategies, time management, and safe work practices.

• Communicate clearly and efficiently with physicians, caregivers, team members, and community partners.

• Maintain knowledge of hospice nursing best practices and pursue continued professional development.

• Represent Carroll Hospice and Carroll Hospital in the community through liaison activities.

Required Qualifications:

• ActiveRegistered Nurse (RN)license through the Maryland Board of Nursing (MBON)

• AHA BLS Providercertification

• Validdriver’s licenseand ability to pass fleet safety requirements

• Reliable transportation
