Purpose
The Senior Ophthalmic Patient Care Coordinator is responsible for overseeing patient care coordination activities, ensuring efficient communication between patients, healthcare providers, and other institutions. This role involves managing complex cases, providing advanced patient education, and leading a team of coordinators. Reporting to the Clinical Program Manager, the Senior Coordinator works closely with physicians, and administrative staff.
Responsibilities
Patient Communication
Manages patient inquiries via phone, email, electronic health record portal, and face-to-face interactions.
Provides detailed information on health conditions and treatment plans.
Coordinates communication between patients and healthcare providers.
Appointment Scheduling
Schedules patient appointments with doctors and specialists.
Manages the calendar to avoid conflicts and ensure timely visits.
Sends reminders and follow-up messages to patients.
Patient Education
Educates patients on their health conditions, treatment options, and preventive care.
Provides resources and materials approved by healthcare providers.
Conducts follow-up to ensure understanding and compliance.
Administrative Tasks
Maintains patient records, including contact information and medical history.
Processes prior authorizations and patient assistance program applications.
Coordinates and documents all referral processing steps throughout multiple stage
Quality Improvement
Required Qualifications
High school diploma or equivalent.
3 years of relevant experience in a healthcare setting.
Knowledge of medical terminology and patient care processes.
Excellent communication and organization skills.
Must have high energy level, flexibility, and a desire to learn.
Preferred Qualifications
Associate's degree in healthcare administration or related field.
5 years of experience in patient care coordination.
Experience with electronic health records (EHR) systems.
Bilingual (English/Spanish)
Certified Ophthalmic Assistant or Certified Ophthalmic Technician
Relevant education and experience may be substituted as appropriate.
Salary Range
$48,000 + depending on qualifications
