POSITION SUMMARY:
As a cause-driven leader and integral member of the Community Wellbeing team, the Associate Executive Director, Community Wellbeing,will support the Executive Director in executing the organization’s comprehensive community health strategies. This leadership role is responsible for overseeing and guiding the Y’s Community Health Worker initiatives, with a strong emphasis on advancing health equity, increasing access to care, and addressing the social determinants of health in underserved communities.
The AED, Community Wellbeing will provide day-to-day supervision, support, and professional development for a team of Community Health Workers embedded across multiple Y family centers and community sites. This individual will ensure that the CHW team is equipped to build trust with community members, conduct effective outreach and assessments, connect individuals to vital resources, and support chronic disease prevention and self-management programs.
In alignment with the Y’s Health Equity Strategy, the AED, Community Wellbeing will also play a key role in developing and refining workflows, evaluating impact, and sharing outcomes to demonstrate effectiveness and secure sustained investment. This includes supporting data collection and reporting, ensuring compliance with grant requirements, and identifying areas for quality improvement.
Additionally, the AED, Community Wellbeing will assist the Executive Director in cultivating and managing partnerships with hospitals, health systems, academic institutions, government agencies, and community-based organizations. These collaborations are critical for expanding access to services, co-designing innovative health initiatives, and improving long-term health outcomes for the populations we serve.

Implements and evolves the organization’s healthy living strategy
Lead the organization’s health equity strategy, focusing on initiatives that promote health equity within the community
Coordinates each region’s community health needs assessment
Implements and scales the Y’s Community Health Workers initiative, ensuring its alignment with the organization’s strategy and community needs
Food Security
Health Partnerships
Cultivate and steward strategic relationships with organizational and healthy living partners
Identify and engage prospective partners to enhance community health initiatives
Establish and execute Memorandums of Understanding (MOUs) with partners
Data-Driven Prevention Interventions
Leverage data analysis to develop and implement evidence-based prevention interventions
Interpret data insights to enhance the effectiveness of community health programs
Develop systems for tracking outputs and outcomes data for community health initiatives
Supervision and Leadership:Lead, train, and support Community Health Workers (CHWs), ensuring they effectively fulfill their roles in providing community-based health services.
Program Management:Oversee SNAP outreach, food access programs, harm reduction, and population health initiatives.
Performance Monitoring:Conduct regular performance evaluations, provide coaching, and offer professional development opportunities for CHWs.
Compliance and Reporting:Ensure compliance with grant requirements and maintain accurate records for audits, grant reviews, and internal assessments.
Data Collection and Evaluation:Monitor program effectiveness through data collection, reporting, and program evaluation.
Community Engagement:Build and maintain relationships with local organizations, healthcare providers, and community agencies to enhance service delivery and resource access.
Client Services:Support CHWs in managing client caseloads, conducting intakes, providing referrals, and offering advocacy and care management.
Outreach and Event Coordination:Lead community outreach efforts, organize events, and promote the organization’s health initiatives.

QUALIFICATIONS:
EDUCATION
Bachelor's degree
Minimum of five years of related experience as a coordinator of people, operations and/or activities
Relevant project management experience
Exceptional communication skills (oral and written)
Highly collaborative work style
Highly organized and able to multitask
Excellent relationship-building and business development skills
Comfortable and confident speaking on behalf of the organization and articulating the Y's mission and priorities to a wide range of groups and individuals
Strong program design skills
Comfortable with database management and tracking systems
EXPERIENCE
Five or more years of management experience with a strong commitment to the Y philosophy and values and significant experience in nutrition and exercise programs.
Experience in member service & engagement and quality management.
Multi-site management experience is required.
