Location Detail: 425 Post Road Fairfield (10230)
In collaboration with attending surgeons, this role is responsible for providing skilled health care to patients in a variety of clinical settings and facilitating clinical management of patients by collaborating with attending surgeons, clinical nurses and other members of health care teams.
The scope of responsibility is to assess, plan, evaluate and treat human responses to actual or potential surgical problems utilizing appropriate practices, standards, protocols and guidelines.
·Provides direct patient care for in-office consultations, including assessment, workups, triage, procedures, emergency work-ins, and other patient care activities
·Obtains and documents health history, performs and documents physical exam, and orders and interprets diagnostic tests
·Provides counseling to patients regarding diagnosis and expected course of treatment
·Collaborate with physicians to provide appropriate direct care interventions within scope of practice
·Collects, organizes, records, and discusses data as appropriate and uses sound clinical judgment based on best practice standards and evidence-based medicine
·Prescribes pharmaceuticals according to patient needs and conducts appropriate patient follow-up & outreach
·Ensures timely retrieval of laboratory and ancillary studies and diagnostic consultation reports
Shift – 40 hours per week.
Primary Location: 425 Post Road, Fairfield - Provider will be expected to see patients at multiple sites once training is completed, as we have satellite offices.
Training Location: Training for this position may occur at the Fairfield office or at another Hartford HealthCare location within Connecticut.
This role has the potential to evolve, possibly including rounding at SVMC and assisting providers in the OR. However, the core responsibility will be focused on seeing patients in the outpatient clinic.
Qualifications
·Eligibility to be licensed in Connecticut as a Physician Assistant (PA), or Advanced Practice Registered Nurse (APRN), is required.
·Eligibility to be certified as a Physician Assistant (PA), or National Board Certification as a Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), or Adult Gerontology Primary Care Nurse Practitioner (AGPCNP) is required.
·Current State of CT PA/APRN license with prescriptive authority and current DEA number & 100% attendance to maintain annual mandatory skill updates
·2 years of experience in general surgery/colorectal surgery, ambulatory surgery or acute care setting for surgical care preferred
