A healthcare facility in Fort Edward, NY is seeking a Director of Social Work (DSW) or Director of Social Services.
We’re looking for an experienced professional to lead the Social Services department. This role includes staff supervision, care planning, resident support, and coordination of discharge planning. Occasional community outreach may be part of the responsibilities.
Responsibilities:
Oversee the Social Services department
Supervise and support department staff
Guide assessments and individualized care plans
Assist residents and families with adjustment and discharge planning
Represent the facility in community outreach efforts as needed
Apply sound judgment and creativity to achieve departmental goals
Requirements:
2+ years of management experience in a long-term care or healthcare setting
MSW and LSW required
Strong understanding of state and federal regulations
Excellent leadership and interpersonal skills
