Full Time position on the Inpatient Wound Team providing nursing consultation to a diverse population with acute wound, ostomy and continence issues.

Job Summary:
The Wound Care/Ostomy Registered Nurse has the responsibility and accountability for utilizing the nursing process in the prevention and treatment of acute and chronic wounds, ostomy and continence care. Functions in roles of direct expert care, clinical resource, consultant, staff and patient educator. Applies the nursing process within the context of the organization's Framework for Professional Practice. Collaboratively manages transitions across the continuum of care, to assist patients or populations in achieving or maintaining an optimal level of health and achieving best outcome. Recognized as professional role model, mentor and clinical resource. Reports to Program Manager, Inpatient Wound Team.

Job Responsibilities:
• Provides direct care to patients with complex acute and chronic wounds, urinary and fecal diversions, stoma care, fistulas, percutaneous tubes, pressure ulcers, lower extremity wounds, surgical wounds, incontinence/moisture associated dermal conditions and others.

• Provides collaboration and consultation to the health care team in developing and implementing plans used in the identification and management of patients with potential or actual alteration in skin integrity and stoma /fistula care.

• Provides appropriate education to patients, families, caregivers, and staff regarding wound and skin care, ostomy care, percutaneous tubes, and draining wound/fistulae management.

• Evaluates patient response to treatment in the progress of wound healing, ostomy self-care skills training and other outcomes; and adjusts plan of care as indicated.

• Uses protocols and evidence-based guidelines for appropriate and cost-effective use of therapies, products and equipment to prevent and treat skin/wound conditions and ostomy /fistula management.

• Actively participates in orientation, training and champion programs, Quality improvement activities, data analytics and policy /procedure development.

• Provides accurate and timely written documentation of care in the electronic medical record.

• Joins others on the Inpatient WOCN Team at Hartford Hospital.


Qualifications


• BSN Required.
• Graduate of a WOCNCB accredited education program with current Wound / Ostomy /Continence certification preferred.
• Exceptions to the educational requirement will be evaluated on an individual basis. A strong candidate in the process of completing the educational requirement will be considered.
• Recent acute care experience preferred. Weekend coverage required.
