We are currently seeking: - Part Time or Per Diem RNs Case Managers (flexible schedule)
Job Requirements: - • Comprehensive knowledge and skill in current nursing practice.
- • Knowledge of the concept of team and primary care nursing and the implementation of both models of care delivery.
- • Knowledge of the OASIS process.
- • Knowledge of the community resources.
- • Ability to make meaningful observations, evaluations and reports.
- • Ability to recognize the needs and concerns of patients, caregivers, and the multidisciplinary team to maintain therapeutic relationships.
- • Ability to provide skilled nursing care based on scientific principles, the nursing process, basic behavioral concepts, and intelligent professional judgment.
- • Valid driver's license.
