Schedule: Monday- Friday, 8:00am-4:30pm

The Registered Nurse serves as a leader of the healthcare team at Novant Health by establishing an authentic personalized relationship with the patient and their chosen support system, collaborating with physicians and other team members to assess, plan, implement, and evaluate an individualized plan of care that promotes optimal health.The RN will be completing Chronic Care Management visits and Medicare Annual Wellness Visits with patients virtually unless the patient wishes to come in person. Both of those visits are 30-minute visits where the RN would be the provider. This RN has availability for 16 visits per day. CCM caseloads are upwards of 80 patients.

• Education: 4 Year / Bachelors Degree, preferred. Graduate of an accredited school of nursing required. If your RN nursing license has beeninactive, retired or lapsed for five years or more and you have not been licensed in another state during the last five years, successful completion of a Board approved refresher course is required prior to reactivating or reinstating your license. In order to be considered for current licensure status, you must apply for reinstatement within one year of completing the refresher course.
• Experience:One year relevant experience, preferred.
• Licensure/Certification:
  • Current RN license in appropriate state, required.
  • ACLS, PALS, BAT and DOT, preferred.

• The perfect candidate for this position will be a self-starter, self-motivated RN who is ready for a change from traditional bedside nursing and is ready for a more telephonic, sit down, care manager role.
