Position Summary:

The Director of Rehabilitation (DOR) is responsible for managing the daily operations of the rehabilitation department within a skilled nursing facility in accordance with Colorado state and federal regulations. The DOR ensures the delivery of high-quality physical, occupational, and speech therapy services that promote resident recovery, independence, and well-being. This role includes clinical leadership, compliance management, staff supervision, and collaboration with the interdisciplinary care team.

Essential Responsibilities:

  • Direct and coordinate the rehab team including PTs, OTs, SLPs, and therapy assistants

  • Ensure all therapy services comply with Colorado Department of Public Health & Environment (CDPHE) and CMS regulations

  • Monitor and manage staff productivity, caseloads, and outcomes under PDPM guidelines

  • Conduct evaluations and treatments as needed to support residents and team workflow

  • Lead care plan meetings and collaborate with nursing, MDS, and social services

  • Maintain timely, accurate documentation in alignment with payer requirements

  • Support staff development through training, mentorship, and competency reviews

  • Maintain adequate therapy staffing schedules and handle recruitment if needed

  • Participate in audits, surveys, and quality assurance activities

Qualifications:

  • Active Colorado license as a Physical Therapist (PT) or Physical Therapist Assistant (PTA)

  • Preferred 2 years of experience in long-term care or skilled nursing

  • At least 1 year of supervisory or management experience

  • Strong knowledge of PDPM, Medicare Part A/B billing, and EMR systems (e.g., PointClickCare, NetHealth)

  • Excellent leadership, communication, and organizational skills

Preferred Qualifications:

  • Experience with survey readiness and post-acute care transitions

  • Familiarity with Colorado-specific SNF regulations and compliance standards

  • BLS/CPR certification

Working Conditions:

  • Primarily on-site at a skilled nursing facility in Colorado

  • May involve physical activity such as assisting residents or moving equipment

  • Occasional weekend or on-call availability as needed
