Summary of Position: To coordinate care and provides skilled physical therapy to an assigned group of patients in the home setting. To provide supervision and direction to the Physical Therapist Assistant (PTA) providing care in the certified home care. This position reports to the Clinical/Therapy Manager
Essential Functions:
Conducts initial OASIS and ongoing assessment of the physical, psychological and environmental needs of the patient at the start of care, initial PT evaluation and during each visit
Acts as a case manager and determines the needs for other services, notifies Clinical/Therapy Manager, team members and Scheduler of the patient’s needs
Completes PTA supervisory visits per agency policy/Medicare regulations
Provides full report to clinical manager and appropriate clinical staff to ensure services are provided as needed. Monitors insurance authorizations as appropriate
Participates in mandatory meetings, in-services and case conferences as required by HomeCare Maryland’s policy
Completes all documentation per HomeCare Maryland’s policy, state and Medicare regulations
Maintains and reviews medical records, provides care based on established plan of care and physician orders
Conducts the admission/recertification/resumption of care comprehensive assessment, with OASIS data items and medication review
Adheres to HomeCare Maryland’s policies and procedures
Works in a professional and collaborative manner with physicians, team members and office staff
Ongoing interdisciplinary assessment of the patient
Development and evaluation of the plan of care in partnership with the patient, representative (if any) and caregivers
Requirements
Current and valid Maryland Physical Therapy license
Current American Heart Association BLS for Healthcare provider certification Graduate from an accredited Physical Therapy program
Prior experience in home care preferred
Prior managerial experience preferred
1-3 years’ experience acute care experience as a physical therapist
Fleet safety requirement and a valid driver’s license and auto insurance – travel required
