Job Overview:

The Transitions of Care Program Clinical Navigator is responsible and accountable for assuring the delivery of quality health care services to the Transitions of Care patients. The program supports the needs of the Transitions of Care patients by design, management and implementation of the Transitions of Care Program that standardizes care and facilitates continuous improvement to achieve maximum quality outcomes throughout the continuum of care. The Clinical Navigator works with the interdisciplinary team in the development, implementation and evaluation of data collection, analysis and reports according to established clinical practice guidelines and standards. The Clinical Navigator will assist in professional education programs as requested. The Clinical Navigator will provide support and resources to the Transitions of Care patient population.

Qualifications:

Required:

  • BSN
  • Graduate of an accredited school of nursing required
  • A minimum of three (3) years experience, with at least two (2) years in Med-Surg and/or Outpatient Ambulatory Care

Preferred:

  • Knowledge of standard of care treatment for transitions of care
  • Strong med-surg background
  • Certification in Care Coordination

Certifications and Licenses Required:

  • Active NJ RN license or compact RN license with NJ endorsement
  • BLS from the American Heart Association
  • ACLS from the American Heart Association

Scheduling Requirements:

  • Day Shift, 8:30a - 4:30p (variable and not guaranteed)
  • Full-Time, 37.5 hours
  • Monday Friday

Essential Functions:

  • Develops and implements policies/ procedures and clinical practice guidelines that guide and support the provision of services related to Transitions of Care disease processes
  • Coordinates the research and development of clinical outcomes data for the Transitions of Care population
  • Maintains process for clinical data collection, analysis, trending and communication of results
  • Reports Transitions of Care data to appropriate committees
  • Gathers, maintains and analyzes statistical information and assists in its interpretation of practical application
  • Serves as co-leader of the Transitions of Care Performance Improvement Team
  • Coordinates with the Quality Resources Department to ensure acceptable sampling size for chart abstraction and timely communication of results of abstractions
  • Maintains a positive collegial relationship with department heads, physicians, and patients
  • Maintains clinical knowledge to assist clinicians in analysis of clinical data
  • Remains knowledgeable of new procedures and practices in Transitions of Care
  • Benchmarks customer satisfaction outcomes utilizing Press Ganey and HCAHPS surveys
  • Networks and benchmarks with professional organizations to ensure the institution is meeting standard of care
  • Maintains the confidentiality, security and integrity for the management of associate and patient information according to Newark Beth Israel Medical Center policy and procedures
  • Serves as a resource person for staff and other members of the Transitions of Care Team
  • Networks with community organizations regarding Transitions of Care education
  • Collaborates with appropriate affiliates to leverage opportunities and provide seamless customer relationship management
