Level of Care Assessor/ OTR
Position Summary:
The Level of Care Assessor is certified by the state to perform a comprehensive assessment that determines if an individual meets a nursing home level of care and qualifies for PACE services.
Qualifications and Requirements:
Education:
Bachelor’s degree in a health-related field
Experience:
Minimum of 1 year working with a frail or elderly population
Licensure/Certifications:
Valid Driver’s License, proof of auto insurance and consistent and reliable usage of a motor vehicle.
Certification with the National Board for Certification in Occupational Therapy (NBCOT) highly preferred.
Skills and Knowledge:
- Problem solving abilities
- Excellent communication and interpersonal skills
- Basic computer skills
- Knowledge of office equipment (fax/copier)
- Proficient computer skills (word processing, spreadsheets, database)
- Data entry
- Assessment skills
Medical Clearance:Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
