Job Description Summary
The Registered Nurse (RN) with Homecare is a professional caregiver who assumes responsibility and accountability for assessing, planning, implementing and evaluating care of Home Health patients and their families in their home setting. The RN provides the best clinical practice in an environment of caring and healing while working autonomously in the home. Utilizing the nursing process, the RN delivers care in compliance with home health policies and procedures, legal and regulatory standards. The RN utilizes the nursing process by use of technology, therapeutic intervention, evidence-based practice, and coordination of care with other health team members.

Minimum Qualifications:

  • Current RN License in state of practice.
  • Basic Life Support (BLS) Certification for healthcare providers.
  • Current driver’s license, insured and reliable transportation and an acceptable driving record
  • Ability to communicate effectively both verbally and in writing.
  • RNs with less than 12 months of working experience as an RN prior to joining Intermountain must obtain their BSN within five years of their start date.

Preferred Qualifications:

  • Bachelor's Degree in Nursing (BSN)
  • Two years working as an RN
  • Previous experience in an Acute Care, Home Health, or Hospice setting

Physical Requirements:

Physical & Technical Requirements:

  • Visual & Clinical Assessment: Must continuously evaluate and respond to patient needs, reviewing medical labels, operating monitors, and identifying essential equipment and supplies for home-based care.
  • Clear & Effective Communication: Engages with patients, families, and healthcare providers to deliver compassionate and accurate care. Must be able to hear and respond swiftly to alarms, verbal cues, and urgent situationsespecially during emergencies.
  • Precision & Dexterity: Requires fine motor skills to handle delicate medical equipment, administer medications, and perform documentation efficiently. Frequent computer use is necessary for patient records, accessing care instructions, and coordinating home visits.
  • Physical Strength & Mobility: Must safely lift and transfer patients using proper techniques, while also bending, retrieving, and carrying supplies or medical equipment of varying weightsincluding heavy items.
  • Active Movement & Navigation: Regularly assists with patient mobility, transporting equipment, and managing supplies. Often required to maneuver through compact or crowded home environments with obstacles like furniture or power cords, as well as ascend or descend stairs and uneven surfaces to reach patients.
  • Safe & Attentive Driving: Responsible for traveling between patient locations, requiring sustained focus, ability to interpret road signs, and adherence to safe driving practices.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$34.53 - $52.25
