The Hospice GIP Supervisor/Acute Care Navigator is responsible for direct interaction with patients as well as supervision of GIP RNs and Acute Care Navigators in collaboration with the DCS. As an Acute Care Navigator, this position will provide effective placement of home hospice care for patients located throughout the hospital and will work with hospital staff and the General Inpatient (GIP) team to quickly identify potential hospice patients and work to get those patients prepared for a successful transition to home-based hospice care. Functions as an expert in home-hospice capabilities and answers questions from providers, patients, and their families to ensure that the hospice program is well understood and that successful hospital to home transitions occur wherever possible so that patients benefit from hospice while simultaneously reducing acute-care burden. As GIP Supervisor, under the direction of the DCS, ensures staff development, hiring, supervising and evaluating team members.

As Acute Care Navigator:

  • Present home hospice care to patients and families located throughout the hospital to understand the hospice benefit and desire to enroll in home hospice care.
  • Help facilitate referral and transition of patients from hospital to home hospice
  • Evaluate inpatient and emergency department patients for potential hospice eligibility.
  • Evaluate GIP patients for the potential transition to home hospice care.
  • Be visible and available in the Emergency Department (ER) and inpatient units to build relationships with teams in those areas to build awareness of hospice care and the relationships to call the navigator in early to assess potential hospice eligibility.
  • Coordinate closely with ER and inpatient social work and case management teams.

As GIP Supervisor:

· Responsible for the supervision of the GIP team, including coaching, teaching, mentoring, employee engagement, staff rounding, regular 1:1 indirect supervision, onboarding new staff, and contributing to performance evaluations.

· Support continuous improvement efforts, Annual Improvement Priorities, and implementation of “best practices” to ensure clinical excellence.

· Provides appropriate feedback to team members regarding any concern/issues that may arise. Teach/coach and mentor staff supporting their development of confidence, skills and competencies.

· Provide clinical supervision in area of specialty as required and oversight of team to ensure effectiveness of ethical and professional care.

· Monitor performance of GIP Team and report to DCS on productivity; performance; problems/issues/concerns of team.

· Ensure staff follow standards of documentation and provide feedback and coaching on the documentation with the GIP Team.


Qualifications


Education

Bachelor’s degree in nursing, Master’s preferred

Experience

Home-based hospice care experience required. Five+ years social work or nursing experience within a hospice agency or related community health program which included care of the sick at home preferred

Licensure, Certification, Registration

Current discipline licensure to practice in the State of Connecticut.
