Registered Nurse (RN) – Home Health (Weekends Only)

About the Role A leading healthcare provider in the western United States is seeking a compassionate and dedicated Registered Nurse (RN) to join its Home Health team in Salem, OR. As part of a mission-driven organization committed to excellence in patient care, you’ll play a vital role in supporting patients and families with skilled nursing services in the comfort of their homesspecifically during weekend coverage.
This is a weekend-focused position ideal for RNs who are looking for meaningful patient engagement, autonomy in care delivery, and flexible scheduling within a supportive and collaborative team environment.
Key Responsibilities Perform detailed assessments of patients and their families to determine home care needs.
Conduct comprehensive physical assessments and record patient medical histories.
Develop and continually revise individualized care plans in collaboration with patients and families.
Coordinate discharge planning and ensure all needs are addressed for successful transition.
Provide health education and counseling to patients and their support systems.
Deliver care with compassion, clinical skill, and professional accountability.
What You’ll Need Current and valid RN license in the state of Oregon.
At least 1 year of recent acute care experience in an institutional setting.
Basic Life Support (BLS) certification, including hands-on CPR.
Valid driver’s license and reliable insured vehicle for home visits.
Preferred: Bachelor’s Degree in Nursing and 1 year of home health experience.
Compensation & Benefits Pay Rate: $55 – $60 per hour, based on experience
