Summary

Under limited supervision, the Clinical Expert (CNE) applies influence across the three spheres of patient/family, staff, and system to transform healthcare delivery. The CNE maintains up to date and evidence based knowledge in area of expertise. Collaborates with other interdisciplinary/interprofessional health care professionals and service providers to ensure seamless, quality care across the continuum. Exemplifies the Professional Practice Model. A key success factor is the ability to work collaboratively with all levels within the organization, across the LifeBridge system, and with our community partners. This position works under the direct supervision and direction of the assigned Director.
Direct interaction with patients and families to promote health or well-being and improve quality of life. Consultation & direct patient care when advanced skills are needed. Liaise between the providers and staff to facilitate continuity of care. Based on assessments across the patient, nurse and system spheres of influence, determines potential risks to patient safety, autonomy, and quality of care. Synthesizes assessment data, based on advanced knowledge and experience, using critical thinking and clinical judgment to consider differential diagnoses for clinical problems amenable to CNS intervention. Designs strategies, including advanced nursing therapies, to meet the multifaceted needs of complex patients and populations. Provides leadership for collaborative, evidence and best practice based revision of plans of care to advance population specific outcomes. Promotes development of clinical reasoning in healthcare team members (e. g. nursing staff, medical staff, other healthcare providers) and provides coaching to develop, maintain and enhance clinical skills, independent problem solving, systems thinking and decision making. Provides advanced nursing care to patients as appropriate. Documents consultation and provided care in patients’ medical record in accordance with Hospital and Nursing policy. Provides appropriate patient and family education.
Experience:
3-5 years
Education:
Required: Bachelor of Science in Nursing
Preferred: Master of Science in Nursing
Licenses And Certification:
Required: Basic Life Support and Registered Nurse License - Current Maryland license or eligibility to obtain Maryland license
Preferred: Advanced Cardiovascular Life Support ; APRN-Clinical Nurse Specialist License
