Outreach Ambassador
JOB SUMMARY
Under the direction of the Director of Outreach and Engagement (OED), is responsible for generating referrals into the PACE program and educating the community through community outreach, networking, and marketing activities in the community. Promotes and maintains high-quality services according to BoldAge values, PACE principles and best practices.
Develops and cultivates the PACE organization’s network of business, healthcare, and community relationships by:
- Developing a deliberate and organized approach to identifying and engaging with referral partners within the community
- Actively participating in community groups
- Presenting at community events about PACE and care of older adults
- Cultivating and maintaining positive and constructive relationships with healthcare providers, senior service providers and other referral sources
- Establishing positive relationships with potential participants, their families, and caregivers
- Responding timely to all inquiries and commitments
- Evaluating and learning about the needs of the community, especially older adults, and their families, and incorporating learned information into community outreach
Facilitating the assessment and eligibility determination processes by identifying quality referrals and collaborating with other enrollment staff and the interdisciplinary team.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Implement the Outreach and associated activities according to the department’s plan for outreach and engagement
- Utilize all relevant data and knowledge of the service area demographics, competition, past experiences, community needs and national PACE standards in performing outreach activities
- Include a broad range of targeted organizations, groups, and individuals in outreach activities.
- Develop a strategic approach to outreach
- Establish a “person-centered” approach in all outreach and intake activities that optimizes every encounter with a potential enrollee and referral source.
- Maintain a people-first approach that aims to exceed expectations.
- Communicate with, listen to, and interact with participants and their families / caregivers, staff, and all stakeholders on a regular basis.
- Meet and strive to exceed expectations for census enrollment targets.
- Provide individual and group outreach presentations to potential referral sources.
- Maintain consistent, close contact with existing referral sources.
- Develop and cultivate new referral sources.
- Maintain detailed documentation and data. Provide regular reports of all outreach and engagement activities
- Participate in outreach and intake activities. Plan and coordinate open houses/marketing events
- Attends staff meetings and in-service training as required.
- Apply principles of diversity and inclusion in all outreach and engagement activities, communications, and networking.
- Follow all Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
- Protect privacy and maintain confidentiality of all company procedures, results and information about employees, participants, and families.
- Practice Standard precautions andfollowsappropriate Infection Control procedures.
- Maintains safe working environment.
- Participate in and supports the Quality Improvement initiatives.
- Participate in continuing education classes and any required staff and training meetings.
- Maintain professional affiliations and any required certifications.
- Perform other duties as required or requested.
REQUIREMENTS
- High school diploma required.
- A baccalaureate degree in health care administration, business, communications, marketing, or related human services field is preferred.
- Experience in community outreach, intake, enrollment, or marketing preferred. Management experience in a human services organization preferred.
- Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
- Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
